There is no Preferred Provider Network for the Fund's Indemnity Plan. If you elect to enroll in the Indemnity Plan, you may go to the dental provider of your choice. The Plan reimburses you for covered dental services provided to you or your eligible dependents up to the usual, customary and reasonable amount subject to the provisions and exclusions in the Summary Plan Description (SPD).
